Membranous septum length as predictor for permanent pacemaker implantation after TAVI

چکیده
Abstract Background High-degree atrioventricular (AV) block and permanent pacemaker (PPM) implantation represent major complications after transcatheter aortic valve (TAVI). Extension of indication for TAVI towards subjects with lower surgical risk requires to reduce the likelihood requirement implantation. Data on role membranous septum length as potential predictor AV are scarce. Purpose We examined need PPM in a large cohort consecutive TAVI. Methods In 1365 patients without prior who underwent transfemoral TAVI, clinical procedural characteristics were assessed systematically. Based cardiac computed tomography performed was measured orthogonal anulus plane (see figure). Results Median age 81 (IQR 7) years, 50% male. Logistic euroSCORE 12.8 15.7), STS score 3 (2.7). 9,8% had pre-interventional complete right bundle branch (RBBB). 71% received balloon-expandable, 29% self-expandable valve. n=153 (11.2%), necessary due high-degree block. 2.9 mm 2.5mm) versus 4.3 3.2 mm) did not (p=0.061). univariate regression analysis, RBBB (p<0.001, OR 7.8), prosthesis (p=0002, 1.7) (p=0.027, 0.9 per 1 identified significant predictors multivariate all parameters remained significant, including (p=0.009, mm). Conclusion patients, we able confirm independent predictive value length, addition or prosthesis, regarding occurrence post-procedural The results may contribute improved stratification Funding Acknowledgement Type funding sources: None.

Takotsubo Cardiomyopathy after Permanent Pacemaker Implantation
A 78-year-old woman presented to our emergency room because of complete atrioventricular block. The patient experienced significant psychophysical stress during pacemaker implantation due to fear of operation. Acute lung edema occurred four hours after permanent pacemaker implantation. Coronary angiography showed no significant coronary artery stenosis and left ventriculography revealed typical...
متن کاملThe Absence of Atrial Contraction as a Predictor of Permanent Pacemaker Implantation after Maze Procedure with Cryoablation
BACKGROUND The absence of atrial contraction (AC) after the maze procedure has been reported to cause subsequent annular dilatation and to increase the risk of embolic stroke. We hypothesized that the lack of AC could increase the risk of permanent pacemaker (PPM) implantation in patients undergoing the maze procedure. METHODS In 376 consecutive patients who had undergone a cryo-maze procedur...

PURPOSE:To determine the indication for and incidence and evolution of temporary and permanent pacemaker implantation in cardiac transplant recipients. METHODS: A retrospective review of 114 patients who underwent orthotopic heart transplantation InCor (Heart Institute USP BR) between March 1985 and May 1993. We studied the incidence of and indication for temporary pacing, the relationship betw...
متن کاملRoutine chest radiography after permanent pacemaker implantation: is it necessary?
BACKGROUND AND AIMS Chest radiographs (CXRs) are performed routinely after permanent pacemaker implantation to identify pacemaker lead position and exclude pneumothorax. We assessed the clinical value and need for this procedure. DESIGN Retrospective analysis of pacemaker data and CXRs following permanent pacemaker insertion between December 2002 and February 2004. MATERIALS AND METHODS Pos...
متن کاملMarked troponin elevation after implantation of a permanent antibradycardia pacemaker.
INTRODUCTION Transvenous insertion of endocardial leads for permanent pacing is often accompanied by minor myocardial damage, detected thanks to the high sensitivity of cardiac troponins. It is unknown whether higher troponin levels, commensurate with more severe myocardial damage, can be encountered after implantation procedures. METHODS Over a 3-year period, 283 patients underwent an implan...
